Reporting with contact details means that we can offer you direct support and updates on the on incident/concern you report, and where appropriate the University can take formal action.

If you have changed you mind and no longer want to give your contact details, please Report A Concern Anonymously.

Please read the information below before completing the form, this will ensure we receive the relevant information relating to your incident/concern.

• A Mental Health Concern

  Here you can tell us about concerns relating to:

  • Substance Misuse
  • Domestic Abuse
  • Mental Health
  • Sexual Assault
• A Physical Health Concern

  Here you can tell us about concerns relating to a student’s physical health. This could be an issue affecting their mobility, a short or long-term health condition, or admission to hospital for example.

  You will be asked to log under one of the following categories:

  • Ambulance Call
    • Please use this category even if you are not aware of the nature of the illness but are aware of an ambulance call.
  • Health Condition
    • If the condition is related to Mental Health, for example self-harm or an overdose, please use the Mental Health Category.
  • Injury
    • Tell us about any injury or trauma which may affect a students ability to study or where they may need temporary adjustments or advice around accessing study.
  • Other
    • If your concern doesn’t fit any other category, we still want to try and help.
• A Safeguarding Concern

  Here you can raise a safeguarding concern for a vulnerable adult, child, or young person who is associated with the University of Hull through a student, member of staff, or as a visitor to site. Safeguarding is in relation to a vulnerable adult, child, or young person where you may have reasonable cause to suspect that they are experiencing, or at risk of, abuse or neglect and would have care and support needs and where children or adult social care services are, or would need to be, involved.

  Before raising a safeguarding concern please ensure you have familiarised yourself with the Safeguarding Policy.

  All University of Hull Staff are required to complete and keep up to date their safeguarding training in their e-learning portal.

  All University of Hull students are asked to complete the Safeguarding and Prevent module in myJourney as part of their induction.

• A Student Life or Academic Process Concern

  Here you can tell us about concerns relating to:

  • Academic Processes
    • If you have any concerns about any academic process such as mitigating circumstances, or suspending your studies, or need help with them.
  • Accommodation Issues
    • For example housemate disputes, noise complaints, or anything else affecting your living environment.
  • Issues Interfering With Study
    • Any issue which is not a Mental or Physical health condition that is affecting your ability to study.
  • Other
    • If your concern doesn’t fit any other category, we still want to try and help

What Happens Next

Your completed form will be logged to one of our Student Support teams who will review the concern and decide on the best course of action based on the information provided.

If you are reporting a concern on behalf of a person we will acknowledge receipt but due to confidentiality will not be able to share any further details following on from the initial concern.

If you report a concern about another person the details of the concern or the person raising the concern will not be shared with the individual unless agreed.
